Important: Change of Location for Patton Enrollment and New Student Parents' Meeting

Due to construction on Grant Avenue, Patton Enrollment, Patton Placement testing, and the meeting for parents with students new to Patton have been moved to Eisenhower Elementary School.

• The Meeting for Parents of New Patton Students will be held in the auditorium at Eisenhower. The date and time for this activity have not changed:
  (Monday, July 28 at 7:00 PM).
• Patton Enrollment will be held on the lower level of Eisenhower Elementary School. The dates and times for this activity have not changed:
  (Tuesday, July 29 at 8:30 AM for All Patton Students A-L)
  (Wednesday, July 30 at 8:30 AM for All Patton Students M-Z)
• Patton Placement Testing will be held at the original dates/times listed at the bottom of this page
  (Wednesday, July 30th and Thursday, July 31st)

Required Forms for Elementary Students
Each of these forms is to be completed for every student (Grades K to 6):

• Student Enrollment Form
• English to Speakers of Other Languages (ESOL) Survey Form
• Cumulative Health Record Form (completed by parents)
• Technology Acceptable Use Policy Form
• Kindergarten Class Selection Form (if applicable)
• Free or Reduced Meals Application (if applicable, only one form per family is needed)

Completed school physicals are required for enrollment of elementary children age 8 and younger and new to Kansas schools. The physical must have been completed on or after January 1, 2008, for the upcoming school year (SY08-09). USD 207 will accept elementary school physicals on any type of form that your current physician uses, but we have provided the elementary school physical form from Gentry Clinic, Fort Leavenworth, for your convenience. Required Forms for Jr. High Students
Each of these forms is to be completed for every student (Grades 7 to 9):

Sports Physicals for Patton Jr. High Completed sports physicals are required for junior high students that would like to participate in sports or cheerleading within Patton Jr. High School. The sports physical must have been completed on or after May 1, 2008, for the upcoming school year (SY08-09).
